Eligibility Tracks

Three tracks for applications exist and the required documentation depends on the application track chosen. Please note that all supporting documents must be provided in English.All applicants must first pass the IC-OS Board Certification Exam in cardio oncology.

Clinical Excellence

Providers who provide cardio-oncology care in a clinical setting.

 

  • IC-OS Member and Cardio-Oncology Knowledge-Certified
  • CV and One Page Summary of Career Experience
  • At least Three (3) Years of Cardio-Oncology Clinical Experience
  • Copy of Academic Diploma (MD, PhD or Equivalent)
  • Bibliography of Publications, including Three (3) where applicant was a participant and one (1) where candidate was First or Last Author
  • Two Letters of Recommendation, including
Scientific Excellence

Researchers and scientists who focus on the the advancement of cardio-oncology through research or study

  • IC-OS Member and Cardio-Oncology Knowledge-Certified
  • CV and One Page Summary of Career Experience
  • Copy of Academic Diploma (MD, PhD or Equivalent)
  • Bibliography of Publications, including Ten (10) where applicant was a participant and at least Five (5) where candidate was First or Last Author
  • Two Letters of Recommendation, including
Practice Excellence
  • IC-OS Member and Cardio-Oncology Knowledge-Certified
  • Must be listed as Practitioner on Professional Register in Country of Residence
  • At least Three (3) Years of Cardio-Oncology Practice Experience
  • A copy of Nursing, Pharmacy, or Related Diploma
  • CV and One Page Summary of Career Experience
  • Letters Certifying Hospital Appointments
  • Bibliography of Publications, including at least two (2) where applicant was a participant
  • Two Letters of Recommendation, including
    • One (1) from a Board-Certified Member of A Related Professional Society
    • One (1) From a Member of an IC-OS Committee
